Fatores preditores de colapso epiglótico na síndrome de apneia obstrutiva do sono

Resumo

Objetivos: Avaliar se existe uma correlação entre a presença de colapso epiglótico e as variáveis demográficas, antropométricas, polissonográficas e decorrentes da Drug Induced Sleep Endoscopy (DISE).

Desenho do Estudo: Estudo observacional retrospetivo.

Material e Métodos: Doentes com síndrome de apneia obstrutiva do sono submetidos a DISE em 2025. Foi utilizada a classificação de VOTE, sendo considerado apenas o colapso primário da epiglote, caracterizado como presente (E+) se ocorrer colapso total (Classificação 2+ no DISE).

Resultados: Foram incluídos 75 doentes, dos quais 27% (n=20) tinham E+. Verificaram-se diferenças significativas na distribuição do Friedman Tongue Position (FTP) entre doentes com e sem colapso epiglótico (p=0,037). Na regressão logística multivariável, apenas o FTP se associou de forma independente ao colapso epiglótico (OR 3,39; p=0,011), enquanto idade, sexo, índice de massa corporal, perímetro cervical e índice apneia/hipopneia não apresentaram associação significativa.

Conclusões: O FTP foi o único preditor independente encontrado, sugerindo que uma posição mais alta da língua predispõe ao colapso epiglótico.
