Tiroide lingual: um caso clínico e revisão da literatura

A presença de tecido tiroideu ectópico na base da língua denomina-se de tiroide lingual. A maioria dos casos é assintomática, sendo por vezes diagnosticada de forma acidental, por exemplo, no contexto de uma infeção respiratória superior. Neste trabalho apresentamos o caso de uma doente do sexo feminino, 44 anos de idade, com antecedentes de hipotiroidismo, que recorreu ao serviço de urgência por febre, odinofagia, disfagia e otalgia direita. Apresentava edema, exsudado purulento e áreas de necrose na face lingual da epiglote, bandas ventriculares e aritenoides; foi encontrada uma massa na base da língua, na linha média, acima do epiglote, com mucosa de aspeto normal. A tomografia computadorizada cervical confirmou o diagnóstico de supraglotite e mostrou uma imagem nodular hiperdensa com captação de produto de contraste na base da língua, compatível com tiroide lingual; na topografia habitual da tiroide identificou-se apenas uma imagem nodular de pequenas dimensões. O cintigrama com Tc99 confirmou o diagnóstico de tiroide lingual, com uma imagem nodular de maiores dimensões na orofaringe e uma de pequenas dimensões na área topográfica tiroideia.
